Submitted by this question as a link, I did some search queries and found an easy way to do this.
All you have to do is edit the SQLFile.sql located here: <% = your SQL install dir%> \ 90 \ Tools \ Binn \ VSShell \ Common7 \ IDE \ SqlWorkbenchProjectItems \ SQL \ SQLFile.sql
In addition, Ctrl + Alt + T will show you all the other templates in SSMS; and check out the free SQL Server tools while you're on it.
