What is the best way to structure an HTML document for SEO and accessibility?

What is the recommended way to structure html that includes navigation, so it is still optimized and accessible.

For example, if the structure is more similar to the structure of the document:

  • Skip to content (link, hidden CSS)
  • Navigation
  • Content
  • Other content (right panels, etc.)

or

  • Go to navigation (link, hidden CSS)
  • Content
  • Navigation
  • Other content (right panels, etc.)

CSS is obviously used to control layout, but bots and screen programs ignore CSS.
