How can I document tables and fields in SQL Server 2005

I missed this somehow in the process of learning SQL Server.

I would like to be able to document tables / fields / procedures in our SQL Server 2005 environment, and I cannot find a way to do this. Is this possible, or do I need some third-party application?

It sounds like a hassle-free function, so I probably don't notice it.

EDIT: In particular, I'm talking about documenting SQL objects ("this table is used for blah, and it is referenced by foo and bar."). Thanks for all the help - you guys nailed it.
