How to create a simple, secure polling program?

Suppose you are writing a survey application and would like to somehow guarantee that the results are safe from the user's point of view. Simply put, I know what IP you came to, but I want you to sleep well at night, knowing that I know nothing about your answers. I can not store IP in raw form.

I need to guarantee 1 thing, though, that you answer the questions once. Therefore, as soon as your computer comes with some data, I need to admit that your computer has already responded to the survey.

Any suggestions on how to best deal with this?
