Forcing geometry + WGS84, latitude and longitude, but not height?

I am going to perform several calculations using the WGS84 ellipsoid, including: a point in a polygon on a surface, calculations of the distance between points, and conversion to / from Cartesian coordinates. It seems like there is a GIS extension to improve the geometry, which basically does what I need - except that it does not convert heights to / from Cartesian coordinates - only X / Y and Lat / Lon. Did I miss something? Is it possible? Anyone have any suggestions?

I am not sure that the omission is due to the fact that there are very few places in the world where the ellipsoid WGS84 actually corresponds to sea level .

But as for the math itself, I can offer this article , which converts from lat-long height (LLA) to Cartesian, and refers to methods for converting from Cartesian to LLA.
