Magento order form on product page

I am new to Magento and I need some kind of conceptual guidance and / or links. I need to create a custom form that will be part of the page product-info(bottom), and it will collect user input (email address, size, color). Only a certain product will have this function / form (up to the administrator). Therefore, the purpose of this form will be store-admin email about a product that is available in stock but not in the desired size and / or color. Should I customize an existing form Contact usor should I create a new custom form in a new module?
The form should send data to the administrator by e-mail along with the name of the product.

So, first create the .phtml file in the app / design / frontend / [namespace] / [theme] /template/catalog/product/send_request.phtml section

then add this .phtml link to the .xml directory in / app / design / frontend / [namespace] / [theme] /layout/catalog.xml

but you should find on the right side where to place (you will find after several attempts)

Finally, add your php logic to the file send_request.phtml. The form that you have implemented in send_request.phtml will be available on the product review page.
