Speaking from experience, before CRAN accepts the update with another maintainer, they will want to receive an email from the current maintainer explaining their intention to change maintainers. This email must come from the email account specified in the DESCRIPTION file.
After CRAN receives this email, you can edit the DESCRIPTION file to change the maintainer name and contact information, and then send it to CRAN. I would include an explanation of the change accompanying in your submission notes, as well as a link to the email sent to CRAN.
