Changing the window title in SQL Server Management Studio

Question

Is there a way to change the window title of my SQL Server Management Studio to display the current server I'm connected to?

To find out which database I'm connected to, I need to scroll all the way to see the server name, and this is a pain ..

Maybe there is an available plugin or something like that.
