How to create a custom mouse cursor for a website?

I saw several websites with custom mouse pointers, and not with the usual "operating system" cursors. I know that this is very easy to do in flash, etc .; but I am not talking about these sites.

Is it really hard for you to create a custom mouse cursor for a regular web page? I can't remember good websites, but there is one that has a custom, although its pretty rubbish, I was just curious to know what needs to be done to get a custom mouse pointer.
