Looking for a super simple but effective BUG / Feature list that users can vote on

I am looking for some javascript or php code where it allows someone to enter something on the list and allows people to vote (1 vote for IP address or cookie) or speed (not important, good, important).

There would be two lists, one for people would offer functions, and also would increase the speed up and down in the list. Another will be a mistake so that people can vote for the mistake they corrected, or submit a new mistake.

This will only be used by 50 users, so simplicity is required.

Does anyone have a basic script or an example of this? I need a way to keep track of what my users want in the app, and also give them a way to send material that they want to fix.

The idea that inspired this, this page: http://kryogenix.org/bugs/sorttable/ seems manual, but not automated.

This is not a bug tracker in the classical sense, but it allows users to add suggestions and vote for them by other users (as you requested). One list will be free. If you need several lists, you have to pay a fee, but they offer discounts for non-profit organizations.
