Apache FOP can generate PDF files from xsl-fo input. There are commercial (and possibly free that I don’t know) servers, libraries and services that use the xsl-fo extensions for user-filled pdf forms. You can find several using a search query like “xsl-fo“ pdf form. ”Free libraries will certainly be possible because FOP supports extensions, but I don’t know.
