How do websites like Shelfari and Librarything collect book information?

Do they manually add each book with its cover and author or use a scraper?

There is a nice good entry in google answers . This is from 2006, but I do not think that the landscape of the ISBN database market has changed much since then.

I would start by watching Bowker

LibraryThing uses a mix of Amazon API and Z39.50 volunteer libraries to retrieve book data. Each book is added as an LT member, adds it to its directory (which makes it mostly manual).

The third way to get book data is to enter users manually (also manually add covers). Otherwise, the caps come from Amazon.
