Examples of a working implementation of ITestCaseProvider for NUnit?

Question

Does anyone have any working examples of ITestCaseProvider?

The samples I came across are not picked up by any test runs - GUI, Console or Resharper (I put the assembly in all the right places).

I added Debugger.Break for everyone, trying to get the debugger to enter, but nothing so far.

I guess after a walkthrough, if anyone has one, or how to stumble upon it.

I got my ITestCase provider working from this blog here and dropping my build here:

C: \ Program Files (x86) \ NUnit 2.6.3 \ bin \ addins

Unfortunately, it only works in the nunit GUI. ReSharper (v8.2) remains sick without loading it, even if I also posted my assembly here:

C: \ Program Files (x86) \ JetBrains \ ReSharper \ v8.2 \ Bin \ addins

I would like to see a test structure with much easier extension points and simple ReSharper integration.
