Does anyone know why my application bar for Windows Phone 8 does not show the icon that I installed? This is what I get (and yes, I'm sure this is not the icon of my application):

This is what I have so far, and I have run out of ideas about what might be wrong:
- The little icon is png 110x110
- The environment icon is png 202x202
- Both images have the line Type = Content
- Both images have a copy to display property = Copy if new
- The application icon matches the small icon and it appears in the program list.
