As it turned out, this is just a design change in the new version of Android.
Prior to Android 5.0, Lollipop setLargeIcon sets a large icon on the left, and setSmallIcon sets a small icon on the right side, as you would expect.
Starting with Android 5.0, setSmallIcon overlays a small icon in the lower right corner of the large icon. As far as I can tell, there is no way to make a small icon in the right left corner on Lollipop. Please correct me if I am wrong.
