How are Gmail plugins like Rapportive distributed?

I know that Gmail has context and side gadgets, but how are browser extensions like Rapportive? Do they enter their buttons / displays into the browser page? If so, will the browser extension not be violated if Google decides to change the page layout?

Probably use some part of the Gmail Greasemonkey API and use the scripts on the page to use this.
