How to include 2 columns under column heading in html table?

I want to create an html table that should look like this:

I know that I will have to use colspan / rowspan attributes, but how? Can anyone help?

I tried the following:

but it gives me the result like:

Obviously, I need a subtitle in the 3rd heading (dots), how to do this?
