Excellent PivotTable - Multiple Columns

Question

I am trying to create a pivot table in Excel using the following data:

The end result I'm trying to come up with (using the fulcrum) is a summary like this:

However, I cannot figure out how to get Excel to read / add / view all columns. I can create a reference element for each individual field / column, but I cannot figure out how to compile the data together.

Promote mac excel in the picture, but here is how you can set up a pivot table.

Note that row labels will only reflect the categories that are in the column that you put in the row.
