How to create client id and secret for canvas?

I am trying to integrate canvas into my application. so I tried to implement oauth2 stream, I read this link https://canvas.instructure.com/doc/api/file.oauth.html

An OAuth2 token request stream requires an application client identifier and client secret. To get these application credentials, you need to register your application. A customer’s secret should never be shared.

For Canvas Cloud, you can request a customer ID and secret address http://www.instructure.com/partners or contact your account representative.

So, I tried to fill out this form https://docs.google.com/forms/d/1C5vOpWHAAl-cltj2944-NM0w16AiCvKQFJae3euwwM8/viewform to get the client ID and secret.

as soon as I filled out and submitted the form, I get this message

Your request for a development canvas key was submitted to create a structure

So, I can’t find the client ID and client secret after submitting the form.

I hope someone helps me advance in my work.
